Trained and friendly staff provide professional support to help you maintain your independence and stay in your own home. Your care is personalised to meet your needs so you can complete daily tasks, from preparing a meal to managing medication.

Supported living
Support for adults with a physical or learning disability (including complex care needs) so you can live independently in your own home.
Personalised and flexible, you can choose how and when you want support from a couple of hours a week to 24-hour care. Experienced and trained staff will help you build life skills and achieve goals based on your interests and priorities. They can provide a range of support such as:
- personal care
- help with medication
- managing bills and budgeting
- participating in community activities

Reablement to regain skills
Help to get back on your feet, such as after a period of illness or bereavement, so you can stay in your own home as safely and independently as possible.
Reablement support workers will work with you in your home, breaking tasks down into small steps to make them more achievable, for example cooking a meal or going out to your local shops. Your needs will be assessed and support will be put in place to help you reach your own targets.

Community equipment service
Works with partner organisations like the NHS to deliver, install and maintain equipment to help you stay active, comfortable and independent in your own home.
An occupational therapist or district nurse will need to complete an assessment to refer you. Once your needs have been assessed and you’ve been prescribed equipment, we’ll contact you to arrange delivery. We’ll set up any large equipment like beds and mattresses and will show you how to use it.
